Skleroderma hastalarının ağız ve periodontal bulguları ile hastalık tutulumları arasındaki ilişki

Abstract

Amaç: Bu çalışmanın amacı, skleroderma (SD) hastalarının mevcut ağız açıklığını ve periodontal bulgularını tespit etmek ve bu bulgular ile hastalığın alt grupları arasındaki ilişkiyi değerlendirmektir.
Gereç ve Yöntem: Çalışmaya 74 yetişkin (60 kadın, 14 erkek) skleroderma hastası dahil edildi. Çalışma grupları skleroderma tipine (51 Diffüz, 23 lokalize) göre belirlendi. Çalışmaya katılan hastaların klinik periodontal parametreleri (Gingival indeks, plak indeksi ve cep derinliği) kaydedildi. Maksimum ağız açıklığı ve tükürük miktarı belirlendi. Temporamandibular eklem muayenesi yapıldı.
Bulgular: Skleroderma hastalarında ortalama ağız açıklığı 36,6±8,0 mm tespit edildi. Diffüz tip SD hastalarının ortalama ağız açıklığı (33,7±7,3 mm) ve lokalize tip SD hastalarının ağız açıklığı (43,0±5,4 mm) arasında istatistiksel olarak anlamlı bir fark bulundu. Diffüz tip SD hastalarında cep ortalama cep derinliğinin (3,17±0,9), lokalize tip SD hastalarına (2,69±0,6) göre istatistiksel anlamlı düzeyde yüksek olduğu tespit edildi.
Sonuç: Sklorederma yaygın olarak sınırlı ağız açıklığına ve periodontal dokulara etkisi nedeniyle çeşitli oral problemlere yol açabilir. SD hastalarının oral problemlerinin tedavisinde, ağız açma egzersizleri ve düzenli diş hekimi kontrolü önerilebilir. Ayrıca SD tipleri ile periodontal hastalıklar arasındaki ilişkinin etiyolojisi ve patogenezini değerlendiren ileriki çalışmalara ihtiyaç bulunmaktadır.

The relationship between oral and periodontal findings with disease type in scleroderma patients

Abstract

Purpose: The aim of this study is to determine the maximum mouth opening and periodontal findings of scleroderma (SD) patients and to evaluate the relationship of these findings with the subgroups of the disease.
Materials and Methods: Seventy-four adults (60 females, 14 males) with scleroderma were included in this study. Study groups were determined according to the type of scleroderma (51 Diffuse, 23 localized). Clinical periodontal parameters (gingival index, plaque index and periodontal pocket depth) of the patients were recorded. Maximum mouth opening and saliva amount were determined. Temporomandibular joint examination was performed.
Results: The mean mouth opening in patients with SD was 36.6 ± 8.0 mm. A statistically significant difference was found between the mean mouth opening of the diffuse type SD patients (33.7 ± 7.3 mm) compared to the localized type SD patients (43.0 ± 5.4 mm). In diffuse type SD patients, the mean pocket depth (3.17 ± 0.9) was statistically significantly higher than in localized type SD patients (2.69 ± 0.6).
Conclusion: Scleroderma may cause various oral problems due to its wide range of limited mouth opening and its effect on periodontal tissues. Mouth opening exercises and regular dentist check-ups may be recommended in the treatment of oral problems in SD patients. Further studies are needed to evaluate the relationship between SD types and periodontal diseases in terms of etiology and pathogenesis.
