AKUT FAZ GÖSTERGELERİ NORMAL OLAN OBEZLERDE, NÖTROFIL/LENFOSİT ORANI, LENFOSİT/PLATELET ORANI VE FERRİTİN, İNSÜLİN DİRENCİ İLE İLİŞKİLİ MİDİR?

Abstract

Giriş: Obezite, inflamatuar mediatörlerin salınımını uyaran proinflamatuvar bir süreçtir. Bu çalışmanın amacı kilo fazlalığı olan hastalarda obezitenin pratikte sık kullanılmayan inflamasyon göstergeleri ile insulin direnci ilişkisini incelemektir.
Yöntemler: Dahiliye Kliniğine kilo fazlalığı nedeniyle başvuran 18 yaşından büyük, akut faz değerleri normal aralıkta olan 150 hasta ile kesitsel bir çalışma yapılmıştır. Çalışmaya katılımcılar obezitesine göre dört gruba (G1,G2,G3,G4) ayrılarak insülin direnci (HOMA-IR), inflamatuar belirteçlerden nötrofil lenfosit oranı(NLO), lenfosit platelet oranı(LPO) ve ferritin analiz edilmiştir.
Bulgular: G4 grubunda tüm kronik hastalıklar daha fazla saptandı. Özellikle HT, hipotiroidi birlikteliği anlamlı olarak farklıydı(p<0,05). Kilo artışı ile HOMA-IR (3,9±2,2), ferritin 32(3-225) ve NLO (1,8) artmaktadır.
Sonuç: Bu durum obeziteden kronik hastalıklara giden süreçte inflamasyonun bir göstergesi olarak kullanılabilir.

ARE NEUTROPHIL/LYMPHOCYTE RATIO, LYMPHOCYTE/PLATELET RATIO, AND FERRITIN ASSOCIATED WITH INSULIN RESISTANCE IN OBESE WITH NORMAL ACUTE PHASE INDICATORS?

Abstract

Introduction: Obesity is a pro-inflammatory process that stimulates the release of inflammatory mediators. The aim of study is to investigate the relationship between insulin resistance and inflammation indicators of obesity, which are not often used in practice, in patients with excess weight.
Methods: We conducted a cross-sectional study on 150 patients aged 18 years and older, whose acute phase values were within normal range and were admitted to the Internal Medicine Clinic due to excess weight. We divided the participants into four groups (G1, G2, G3, G4) based on obesity and insulin resistance (HOMA-IR), and analyzed inflammatory markers such as neutrophil lymphocyte ratio (NLR), lymphocyte platelet ratio (LPO) and ferritin.
Results: The results showed that all chronic diseases were more common in the G4 group. In particular, there was a significant difference (p<0.05) in the association between HT and hypothyroidism. Weight gain led to an increase in HOMA-IR (3.9±2.2), Ferritin levels (32; range 3-225), and NLO (1.8).
Conclusion: This could serve as an inflammation indicator for the transition from obesity to chronic diseases.
