Kardiyovasküler Hastalığı Olan Bireylerin Gündüz Uykululuk Hali ve Yaşam Kaliteleri Arasındaki İlişki

Abstract

Amaç: Bu araştırmanın amacı, kardiyovasküler hastalığı olan bireylerin gündüz uykululuk hali ve yaşam kalitelerinin değerlendirilmesidir.
Gereç ve Yöntem: Tanımlayıcı tipte olan bu araştırmanın örneklemini, bir üniversite hastanesinin kardiyoloji servisinde yatarak tedavi gören 212 kardiyovasküler hastalığı olan birey oluşturdu. Araştırma verileri Hasta Tanıtım Formu, Epworth Uykululuk Ölçeği (EUÖ) ve MacNew Kalp Hastalığına Özgü Yaşam Kalitesi Ölçeği (MKÖYKÖ) kullanılarak toplandı. Nicel değişkenler ortalama, standart sapma, minimum ve maksimum; nitel değişkenler ise sayı ve yüzde şeklinde sunuldu. İki grup arasındaki fark bağımsız gruplarda t- testi ile üç ve daha fazla gruplar arasındaki fark ANOVA varyans analizi ile değerlendirildi. Değişkenler arasındaki ilişki Pearson korelasyon analizi ile incelendi. Tek değişkenli analiz sonrası anlamlı çıkan değişkenler ile çok değişkenli regresyon analizi uygulandı.
Bulgular: Katılımcıların yarısından fazlasını erkek (%54,2), kronik hastalık öyküsü (%69,8) olan, koroner arter hastalığı olan (%52,4) ve çoğunda gündüz uykululuk hali (%72,6) olmayan bireyler oluşturmaktaydı. Katılımcıların EUÖ puan ortalamalarının 6,46±4,85 ve MKÖYKÖ puan ortalamalarının ölçeğin geneli için 4,11±1,10 olduğu, ölçek puanları arasında negatif yönde (r=- 0,362, p<0,001) anlamlı korelasyon olduğu saptandı. Katılımcıların MKÖYKÖ puanlarının toplam varyansının %23,4’ünü gündüz uykululuk halinin açıkladığı bulundu (F=10,454, p<0,001).
Sonuç: Katılımcıların gündüz uykululuk durumları arttıkça yaşam kalitelerinin azaldığı görüldü. Kardiyovasküler hastalığı olan bireylerin yaşam kalitelerinin gündüz uykululuk hali, cinsiyet ve kronik hastalık bulunma durumundan etkilendiği saptandı. Hastaların yaşam kalitelerin en fazla etkiyen durumun gündüz uykululuk hali olduğu bulundu. Buna göre, kardiyovasküler hastalığı olan bireylerin yaşam kalitelerinde iyileşmeler sağlanabilmesi için yapılacak müdahale araştırmalarında hastaların gündüz uykululuk halinin değerlendirilmesi önerilmektedir.

The Relationship between Daytime Sleeping Status and Quality of Life of People with Cardiovascular Disease

Abstract

Objective: To evaluate daytime sleepiness and quality of life in people with cardiovascular disease.
Material and Method: This descriptive study comprised 212 inpatients with cardiovascular disease in the cardiology service of a university hospital. Research data were collected using the Patient Description Form, the Epworth Sleepiness Scale (ESS) and the MacNew Heart Disease- Related Quality of Life Scale (MHDRQLS). Quantitative variables were mean, standard deviation, minimum, and maximum; qualitative variables were presented as numbers and percentages. The difference between the two groups was determined by a t-test in independent groups and the difference between three or more groups was evaluated with an ANOVA analysis of variance. The relationship between the variables was examined by Pearson correlation analysis. Multivariate regression analysis was applied to the variables found to be significant after univariate analysis.
Results: More than half of the participants were male (54.2%), with a history of chronic disease (69.8%), coronary artery disease (52.4%), and no daytime sleepiness (72.6%). The participants had a negative correlation between their scale scores (r= -0.362, p<0,001) a significant correlation was found. It was found that daytime sleepiness explained 23.4% of the total variance of the participants' MHDRQLS scores. (F=10.454, p<0,001).
Conclusion: It was observed that as the daytime sleepiness of the participants increased, their quality of life decreased. It was determined that the quality of life of individuals with cardiovascular disease was affected by daytime sleepiness, gender, and the presence of chronic disease. It was found that the condition that most affected the quality of life of the patients was daytime sleepiness. Accordingly, it is recommended to evaluate the daytime sleepiness of patients in intervention studies to improve the quality of life of individuals with cardiovascular disease.
