Okul çağı serebral palsili çocuklarda kişisel faktörler, günlük aktivite performansı ve aileye olan etkinin karşılaştırılması

Abstract

Amaç: İşlevsellik, Yeti Yitimi ve Sağlığın Uluslararası Sınıflandırılması ile değerlendirme yapılmaya başlanmasından sonra kişisel ve çevresel faktörlerin aktivite ile ilişkisi önemli hale gelmiştir. Bu çalışmada; okul çağı serebral palsili (SP) çocukların kaba motor etkilenim seviyelerine göre kişisel faktörleri, aileye olan etki ve günlük aktivite performansının karşılaştırılması amaçlandı.

Yöntem: Kişisel faktörler; yaş, cinsiyet, doğum ağırlığı (DA), doğum haftası (DH), anne doğum yaşı bilgileri ile belirlendi. Çocuğun aile üzerindeki etkisi Aileye Etki Ölçeği (AEÖ) ile günlük aktivite performansı; Pediatrik Özürlülük Değerlendirme Envanteri (PÖDE) ile değerlendirildi Kaba Motor Fonksiyon Sınıflandırma Sistemi (KMFSS), El Becerisi Sınıflandırma Sistemi (EBSS), İletişim Fonksiyonu Sınıflandırma Sistemi (İFSS) ve Yeme İçme Fonksiyonu Sınıflandırma Sistemi (YİFSS) ile klinik seviyeler belirlendi.

Bulgular: Çalışmaya yaş ortalaması 10,31±2,07 yıl olan 75 SP’li çocuk katıldı. DA (p=0,005), DH (p=0,006), AEÖ’nin genel etki (p=0,001)) ve sosyal ilişkilerde bozulma (p=0,001) alt başlıklarında ve total etkide (p=0,011), PÖDE’nin kendine bakım (p=0,001), mobilite (p=0,001) ve sosyal fonksiyon (p=0,001) alt başlıklarında KMFSS seviyeleri arasında anlamlı fark bulundu.

Tartışma: SP’li çocuklarda KMFSS seviyesine göre kişisel faktörler, günlük aktivite performansı ve aileye olan etki, değişmektedir. Daha düşük motor fonksiyona sahip çocuklar (KMFSS 4-5) daha çok risk altındadır. Rehabilitasyon uygulamalarında aktivite performansının artırılması ve aileye olan etkinin azaltılmasını sağlayacak, çocuk ve aile odaklı yaklaşımların tercih edilmesi önerilir.

Comparison of personal factors, everyday activity performance and impact on family in school aged children with cerebral palsy

Abstract

Purpose: The relationship between personal and environmental factors and activity has become important after assessment has started with The International Classification of Functioning, Disability and Health. In this study; it was aimed that to compare personal factors, impact on family and everyday activity performance of school age children with cerebral palsy (CP) according to gross motor impairment levels.

Methods: Personal factors were determined with age, gender, gestational age (GA), gestational weight (GW), maternal birth age information. Everyday activity performance was evaluated with Pediatric Evaluation of Disability Inventory (PEDI), Child’s impact on family was evaluated with the Impact on Family Scale (IPFAM). Clinical levels were determined by Gross Motor Function Classification System (GMFCS), Manual Ability Classification System (MACS), Communication Function Classification System (CFSS) and Eating and Drinking Ability Classification System (EDACS).

Results: 75 children with CP mean age of 10.31±2.07 years included the study. GW (p=0.005), GA (p=0.006), IPFAM subdomains familial/social impact (p=0.001) and personal strain (p=0.001) and total impact (p=0.011), PEDI subdomains self-care (p=0.001), mobility (p=0.001) and social function (p=0.001) were significantly different between GMFCS levels.

Conclusion: In children with CP personal factors, everyday activity performance and impact on family change according to the GMFCS levels. Children with lower motor function (GMFCS 4-5) are at greater risk. In rehabilitation practices, it is recommended that child and family-oriented approaches should be preferred to increase activity performance and reduce the impact on the family.
