Cerrahi sonrası iyileşmeyen kronik anal fissürlü hastalarda Botoks uygulaması başarılı mıdır?

Abstract

Amaç: Cerrahi sonrası iyileşmeyen kronik
anal fissür hastalarında botoks uygulamasını ve sonuçlarını retrospektif olarak irdelemeyi
amaçladık.

Gereç ve Yöntem: Kliniğimize Ocak 2016 – Ocak 2018 tarihleri arasında başvuran,
kronik anal fissür tanısı ile lateral internal sfinkterotomi uygulanmış ve değerlendirme sonrası
iyileşmeyen anal fissür tanısı alan hastalar retrospektif olarak
incelendi. Postoperatif inkontinans varlığını
araştırmak için Wexner skorlama sistemi kullanıldı. Anal sfinkterlerin
değerlendirilmesi için tüm hastalara Endoanal
Ultrasonografi yapıldı.

Bulgular: Toplam 36 hastanın, 22’si (%61,1) erkek, 14’ü (%38,9) kadındı. Yaş
ortalaması 36,2 idi. Endoanal ultrasonografi'de 12 (%33,3) hastada superfisiyal
eksternal sfinkterotomi ve intakt internal anal
sfinkter, 24 (%66,7) hastada inkomplet
internal sfinkterotomi saptandı. 18 (%50)
hastada inkontinans mevcuttu. 16 (%44,4) hastada sadece gaz, 2 (%5,6)
hastada gaz-sıvı inkontinansı mevcuttu. Postoperatif Wexner skorları ortalama
1,2 idi. Botoks işlemi sonrası hastaların 30 (%83,3) hastada iyileşme sağlandı. 6 (%16,7) hastada
iyileşme sağlanamadı. Botoks işleminin başarılı olduğu 6 (%20) hastada nüks
gelişti. Ortalama takip zamanı 11,9 ay
idi.

Tartışma ve Sonuç: Çalışmamızda
iyileşmeyen anal fissür hastalarında botoks uygulaması başarı oranı %83,3
olarak bulunmuştur. İkincil cerrahide endişe yaratan inkontinans ihtimali
nedeni ve %80 lere varan başarı oranları ile botoks uygulaması ikincil
cerrahiye alternatif olarak ön planda düşünülmelidir. Ancak kesin bir şey
söyleyebilmek için daha geniş serilere ihtiyaç olduğu kanaatindeyiz.
