TEMPOROMANDİBULAR EKLEM DİSK DEPLASMANI İLE EFÜZYON İLİŞKİSİNİN MANYETİK REZONANS GÖRÜNTÜLEME İLE DEĞERLENDİRİLMESİ

Abstract

Amaç: Temporomandibular eklem (TME) rahatsızlıklarının önemli radyolojik bulgularından biri efüzyondur. Bu çalışmanın amacı, TME disk pozisyonu gruplarıyla efüzyon arasındaki ilişkiyi manyetik rezonans görüntüleme (MRG) ile incelemektir.
Gereç ve yöntemler: Erciyes Üniversitesi Diş Hekimliği Fakültesi Ağız Diş ve Çene Radyolojisi bölümüne TME rahatsızlığı şikayetiyle başvuran ve MRG kaydı bulunan hastalar bu retrospektif çalışmaya dahil edilmiştir. Sagital T1 ve T2 ağırlıklı görüntüler incelenerek disk pozisyonları ve efüzyon varlığı değerlendirildi.
Bulgular: Bu eklemlerin 59’u normal disk pozisyonu, 46’sı anterior redüksiyonlu disk deplasmanı, 59’u anterior redüksiyonsuz disk deplasmanına sahip olarak belirlendi. Disk grupları arasında efüzyon derecesi dağılımı için anlamlı farklılık bulundu (p<0.01). Normal disk pozisyonu grubunda derece 0 (efüzyon yok) efüzyon izlenme sıklığı diğer gruplara göre anlamlı olarak yüksek bulundu(p<0.01) Anterior redüksiyonsuz disk deplasmanı grubunda ise derece 3 efüzyon izlenme sıklığı diğer gruplara göre anlamlı olarak yüksek bulundu(p<0.01).
Sonuç: TME rahatsızlıklarında ileri disk deplasmanlarına eşlik eden bulgular MRG ile belirlenebilir. Disk deplasmanı safhası ilerledikçe efüzyon görülme sıklığı ve şiddeti artmaktadır.
