Abstract
Amfizematöz piyelonefrit (APN) nadir, hayatı tehdit edici, akut ve ciddi nekrotizan bir enfeksiyondur. Bu hastalıkta genellikle böbrek dokusu, toplayıcı alan veya perinefritik dokular etkilenir. Gaz oluşturan organizmaların etkisi ile enfeksiyon bölgelerinde gaz mevcudiyeti görülür. Hastalık ateş, renal bölgede ağrı, akut böbrek hasarı, septik şok veya ölüme neden olabilmektedir. APN radyolojik bir tanıdır ve tanı için bilgisayarlı tomografi en iyi görüntüleme yöntemidir. Hastalık özellikle diyabet, alkolizm, immünsüpresyon ve üriner yolda obstrüksiyon gibi durumların varlığında daha sık görülmektedir. Literatürdeki APN olgu bildirimlerinde genellikle sol böbrekte olmak üzere tek böbrekte etkilenme bildirilmiş ve tedavi olarak çoğu hastada nefrektomi uygulanmıştır. Bu makalede akut böbrek hasarı ile prezente olan, double j üreteral katater ve antibiyoterapi ile başarılı bir şekilde tedavi edilen bilateral APN olgusu sunulmuştur.
