PLANTAR FASİİTLİ HASTALARDA KALKANEAL EPİN VARLIĞI AĞRI ŞİDDETİ, FONKSİYON VE YAŞAM KALİTESİNİ ETKİLER Mİ?

Abstract

Amaç: Çalışmamızın amacı kalkaneal
epini olan ve olmayan plantar fasiitli olgularda ağrı şiddeti, fonksiyon ve
yaşam kalitesi değerlerini karşılaştırarak fark olup olmadığını ortaya koymaktı.



Yöntem: Plantar fasiit
tanılı yaş ortalaması 43
,6±8,57 olan 25 birey (18 kadın, 7 erkek) çalışmaya katıldı. Bireylere
ait etkilenmiş 43 ayak değerlendirildi ve 23 ayakta
hekim
tarafından ya
pılan radyografi incelemeleri sonucunda plantar fasiite ek olarak
kalkaneal epin belirlenerek kalkaneal epini olan (PF+KE, n
=23) ve olmayan (PF, n=20) plantar fasiitli olgular şeklinde iki gruba ayrıldı.
Olguların sabah uyandıklarındaki ilk adımlarında, palpasyonla ve uzun ayakta
durma sonrasındaki ağrı şiddetleri Görsel Analog Skalası, eklem hareket
açıklığı ve plantar fleksör kas kısalığı gonyometrik ölçüm
, ağrı-fonksiyon-dizilim
AOFAS Arka Ayak Ölçeği
, yürüme performansları 6 dakika yürüme
testi ve yaşam kalitesi WHOQL-BREF ile değerlendirildi. Gruplardan elde edilen
veriler istatistiksel olarak karşılaştırıldı.



Sonuçlar: Grupları arasında yaş
(p=0.353), boy uzunluğu (p=0.768), vücut ağırlığı (p=0.251) ve vücut kütle
indeksleri (p=0.435) açısından fark yoktu. Ağrı şiddeti değerleri, ayak bileği
dorsifleksiyon hareket açıklığı, ayak bileği plantar fleksör kas kısalığı
sonuçları, AOFAS skoru, 6 dakika yürüme testi mesafesi ve WHOQOL-BREF çevresel
skorunda gruplar arasında anlamlı bir fark bulunmazken (p>0.05); WHOQOL-BREF
fiziksel
ve psikolojik
alt bölüm skorları PF grubunda
, sosyal alt
bölüm skoru ise sadece PF+KE grubunda anlamlı derecede yüksekti (p<0.05).



Tartışma: Plantar fasiitte
eşlik eden epin varlığı bireylerin yaşam kalitesini fiziksel ve psikolojik
açılardan olumsuz etkilerken sadece plantar fasiiti olan bireylerde ise sosyal
etkilenme daha fazladır. Erken dönemden itibaren uygulanacak tedavi
yaklaşımlarında yaşam kalitesini arttırmak hedeflerden biri olmalıdır.
