A new method to prevent recurrence in Morgagni hernia surgery

Öz

Amaç: Morgagni hernisi onarımı sonrası nüks gelişimi ciddi bir sorundur. Bu çalışmada morgagni hernisi onarımında kullandığımız yeni yöntemimizi literatür eşliğinde sunmak istedik.
Materyal ve metod: Kliniğimizde 2010 ocak ile 2019 arlık tarihleri arasında morgagni hernisi nedeniyle 14 olgu opere edildi. Hastalar yaş, cinsiyet, başvurudaki semptomlar, tanı yöntemleri, tedavi ve sonuçlar açısından geriye dönük olarak değerlendirildi.
Bulgular: Hastaların 6’sı bayan ve 8’i erkek idi. Ortalama yaş 48 (17-64) idi. Hernilerin tümü sağ tarafta idi. Herniye olan organlar; 5 hastada omentum ve kolon, 9 hastada sadece omentum idi. Ortalama hastanede kalış süresi 5.7 gün idi. Mortalite gözlenmedi. Nüks, takip sırasında tüm hastalarımızın 2’sinde saptandı. Son 4 hastada ekstrakorpereal dikiş kullandığımız yeni yöntem kullanıldı. Bu hastaların hiç birinde nüks saptanmadı.
Sonuç: Sonuç olarak Morgagni hernilerinin onarımında defektin primer onarımı yanında ekstrakorporeal dikişlerinin nüksü önlemede faydalı olacağını düşünüyoruz. Bu yöntem Down sendromlu çocuklarda ve erişkin obez hastalarda özellikle akılda tutulmalıdır.

Öz

Aim: Recurrence after Morgagni hernia repair is a serious problem. In this study, we wanted to present our new method used for repair of morgagnia hernia in the light of the literature.
Material and Methods: Between January 2010 and December 2019, 14 patients with Morgagni hernia were operated in our department. Patients were evaluated retrospectively regarding age, sex, symptoms at admission, diagnostic methods, treatment and outcomes.
Results: 6 of the patients were female and 8 were male. Mean age was 48 years (17-64 years). Hernias were located on the right side in all patients. Herniated organs were: omentum and colon in 5 patients, only omentum in 9 patients. Mean hospital stay was 5.7 days. There were no mortalities observed. Recurrence was detected in 2 of all patients during follow-up. We used our new method of extracorporeal suturing in 4 patients. We did not detect recurrence in any of these patients.
Conclusion: As a result, we think that extracorporeal sutures will be beneficial in preventing recurrence in addition to primary repair of the defect in the repair of Morgagni hernias. This method should be kept in mind especially in children with Down syndrome and adult obese patients.
