Hastanelere Yeniden Yatış Hızlarının Değerlendirilmesi: İç Hastalıkları Servislerinde Yapılan Bir Prospektif Kohort Çalışması

Öz

Hastaneye yeniden yatışlar sağlık bakım hizmetlerinin kalitesi, güvenliği ve maliyeti açısından önemlidir. Yeniden yatış, önceki yatışın ardından, yeniden yatış zaman aralığı denilen belirlenmiş bir zaman aralığında, hastaneye geri dönüp yatış olarak tanımlanabilir. Bu çalışmanın amacı, hastaneye yeniden yatış hızlarının ve yeniden yatışın getirdiği ilave maddi yükün ortaya konmasıdır. Çalışma, prospektif kohort araştırması olarak tasarlanmıştır. Araştırmanın evrenini 1 Şubat 2015- 31 Ocak 2016 tarihleri arasında bir üniversite hastanesinin iç hastalıkları servislerinde yatan hastalar oluşturmaktadır. Veriler hastalardan ve hastane bilgi sisteminden elde edilmiş, SPSS 23.0 ile analiz edilmiştir. Hastaneden taburcu olduktan sonra 30 gün içinde bütün nedenlerden yeniden yatış araştırılmıştır. Yeniden yatış hızı, hastaneye yeniden yatışların sayısını, toplam taburcu sayısına (ölenler hariç) bölerek hesaplanmıştır. Verilerin analizinde ki-kare testinden yararlanılmıştır. Çalışmaya 2464 hasta dahil edilmiştir. Tüm yeniden yatışlar açısından değerlendirildiğinde; aynı hastaneye yeniden yatış hızı %17,9 iken aynı veya başka bir hastaneye yeniden yatış hızı %21,3 olarak bulunmuştur. Aynı hastaneye plansız yeniden yatış hızı %5,2; planlı yeniden yatış hızı %12,7; plansız, ilgili ve önlenebilir yeniden yatış hızı ise %1’dir. Yeniden yatış hızlarının ilişkili olduğu sosyo-demografik ve klinik değişkenler, yeniden yatış türüne göre değişmektedir. Aynı hastaneye genel yeniden yatış hızları; cinsiyet, yaş, eğitim durumu, ikamet yeri, evde bakımına yardım edecek birinin ve düzenli olarak göründüğü bir doktorun varlığı, taburcu tanısı (ICD-10 kodları), Charlson komorbidite indeks skoru, indeks yatış süresi, günübirlik işlem durumu ve taburculuğa hazırlık düzeyinin bilgi düzeyi boyutuna göre istatistiksel olarak anlamlı fark göstermektedir. Yeniden yatışlar için hastanede toplam 2.824.328 TL harcama yapılmıştır. Bu harcamanın %25’i plansız yeniden yatışlar için yapılmış ve %7,1’i hastalar tarafından karşılanmıştır. Yeniden yatışlar için yapılan ortalama harcama tutarı, ilk yatışlar için yapılan ortalama harcama tutarının yaklaşık olarak 1,4 katıdır. Araştırma bulguları, yeniden yatış hızlarının azaltılması gerektiğini açıkça ortaya koymaktadır. Yeniden yatışların azaltılması için öncelikle bütün yeniden yatışların tek bir veri tabanından izlenebileceği, ülke düzeyinde bir sistemin kurulması gerekmektedir.
