Metabolic Monitoring of the Patients with Serious Mental Illness (SMI) in a Community Mental Health Center (CMHC) During the COVID-19 Pandemic
Amaç: Şizofreni ve Bipolar Bozukluk gibi ciddi akıl hastalığı tanısıyla takipli hastaların metabolik izlemleri COVID-19 salgını sırasında sağlık hizmetlerinin kesintiye uğraması nedeniyle aksadı. Bu çalışmadaki amacımız, bir Toplum Ruh Sağlığı Merkezinde (TRSM) takip edilen hastaların başlangıçtaki (pandemi öncesi) ve COVID-19 pandemisinden 12 ay sonraki metabolik parametrelerini karşılaştırmaktır.
Yöntem: Vücut kitle indeksi (VKİ), bel çevresi, yüksek yoğunluklu lipoprotein (HDL), aterojenik plazma indeksi (AIP) gibi metabolik parametreleri başlangıçta (pandemi öncesi) ve pandemi sonrası 12.ayda karşılaştırdık.
Bulgular: VKİ (p<0,001), bel çevresi (p<0,001) ve HDL düzeyleri (p<0,001) hem şizofreni (n=17) hem de bipolar bozukluk (n=17) hastaları için pandemi sonrası 12. ayda başlangıca göre göre anlamlı olarak daha yüksek olarak saptandı. Obezite prevalansı, 12. ayda başlangıca kıyasla istatistiksel olarak anlamlı düzeyde daha yüksekti. AIP düzeyinde istatistiksel olarak anlamlı düzeyde değişiklik saptanmadı.
Sonuç: COVID-19 pandemisi boyunca uygulanan sosyal kısıtlamaların neden olduğu fiziksel hareketsizlik metabolik parametreler üzerinde kötüleştirici etkiye neden olabilir. Hastaların metabolik izlemleri VKİ, AIP, bel çevresi ve açlık glukozuyla yapılabilir. Yaygın aşılama programı ve sosyal kısıtlamaların azalması hastaların metabolik izlemlerinin yeniden başlaması ve gerekli müdahalelerin yapılması için fırsat sunmaktadır. Obezitenin COVID-19 ile ilişkili hastaneye yatış, ağır hastalık ve ölüm riskinde artışa neden olduğu göz önünde bulundurularak hastaların pandemi sürecinde aksayan metabolik izlemleri dikkatle yapılmalıdır.
