Evaluation of the Association of Novel and Conventional Inflammation Indices with Acne Vulgaris Severity: A Retrospective Case-Control Study

Abstract

Purpose: We set out to quantify conventional (NLR, PLR) alongside newer multicellular inflammatory indices (SII, SIRI, AISI) in acne vulgaris (AV), and to determine how closely each tracks clinical severity. Materials and Methods: Records of 230 individuals carrying a clinical AV diagnosis were reviewed retrospectively against 265 controls frequency-matched for age and sex, yielding a study population of 495. Within the AV arm, cases were assigned to mild, moderate, or severe categories. The five indices were derived from routinely obtained blood count parameters and compared across arms. Results: Four of the five indices — NLR, SIRI, AISI and SII — separated the AV arm from controls (p<0.001, p=0.002, p<0.001, and p<0.001, respectively), while PLR did not. The same four rose alongside severity category (p<0.01), though the increment clustered at the mild-to-moderate step and flattened beyond it. Among them SIRI produced the most orderly stepwise pattern (p=0.0001). Severity correlated positively with NLR (r=0.310), SII (r=0.269), SIRI (r=0.235), and AISI (r=0.209). Conclusions: AV is not merely a localized skin disorder but a condition carrying a substantial systemic inflammatory burden. Multicellular novel-generation indices, particularly SIRI and AISI, carry no additional cost beyond a routine complete blood count. They may serve as non-invasive, objective adjuncts to classical markers such as NLR and SII in reflecting systemic inflammation and its association with clinical severity.
