Does Nearinfrared Spectroscopy Really Measure Exact Tissue Oxygen Saturation?

Abstract

Purpose: It is well known that the alterations in circulation during cardiopulmonary bypass have negative effects on cerebral blood flow. Since the usage of Near Infrared Spectroscopy NIRS detect these changes in cerebral blood flow was shown to improve outcome, NIRS has been used in cardiac surgery. But doubts about the reliability of NIRS method and the lack of another method to compare with NIRS limit the widespread use of this method. In our study; reliability of NIRS was aimed to be shown by comparing the saturation values calculated via NIRS with the values calculated mathematically. Patients and Methods: 35 consecutive extubated patients that had undergone elective aortocoronary bypass surgery were enrolled in this prospective study. The saturations were calculated via probes that were placed on the arm. Meanwhile the blood samples from radial artery and brachial vein from the same side with the probe were collected and by the arterial blood gas analysis saturation were mathematically calculated 1/3 of arterial saturaiton + 2/3 of venous saturation . Pearson correlation was calculated between the values calculated by these two methods. Results: Pearson Correlation Coefficient between the saturation levels calculated by NIRS method INVOS and calculated mathematically was found 0,87 p=0,0001. Conclusion: Correlation value of r=0,87 between rSO2 and mSO2 is promising that NIRS can also be used in adult patients for tissue saturation followup as well as children.
