An Unusual Location For a Common Tumor: Basal Cell Carcinoma At The Nostril

Abstract

Basal cell carcinoma BCC frequently occurs at skin of nose, but primary involvement of anterior nares and nasal floor seems to be very rare. A 62-year-old man is presented with an ulcerated plaque on the right nostril which developed 10 years ago and enlarged in time. He denied excessive sun-exposure or trauma to this area. The history was negative for immunosuppressive diseases or therapies. On dermatologic examination, small, pinkish, waxy papules composing a rolled edge were seen on the base and lateral walls of the right nostril. The lesion was extending into nasal cavity, with an indistinct border. Total body skin examination did not show any findings for syndromic BCC. Histopathologic examination revealed a solid type of BCC, involving both skin and mucosal surfaces of the nostril. In several large case series of nasal BCC, involvement of nostrils was exceptionally rare. The cause of this infrequency is not known exactly yet, but is likely to be affected by a relatively low exposure of ultraviolet irradiation and undefined factors.

Burun Deliğinde Bazal Hücreli Karsinom: Olağan Bir Tümör, Olağandışı Bir Yerleşim

Abstract

Bazal hücreli karsinom BHK burunda sık yerleşir, ancak tümörün burun boşluğu tabanı ve burun deliklerinden başlaması çok nadirdir. Altmış iki yaşında bir erkek hasta sağ burun deliğinde 10 yıl önce başlayıp ilerleyen ülserli bir plak nedeniyle başvurdu. Bu bölgeye travma veya aşırı ve uzun süreli güneş teması belirtilmedi. Öyküde immünsüpresyona yol açacak herhangi bir hastalık veya tedavi de tanımlanmadı. Deri muayenesinde sağ burun deliğinin alt ve yan duvarlarında deri renginde ve pembemsi küçük papüllerin oluşturduğu, kabarık kenarlı, ortası yüzeyel olarak ülserleşmiş bir plak saptandı. Lezyon sınırları tam olarak belirlenememekle beraber burun boşluğuna doğru uzanım gösteriyordu. Tüm vücut deri muayenesinde nevoid BHK sendromuna ait herhangi bir bulguya rastlanmadı. Histopatolojik incelemede deri ve mukozayı tutan solid tipte BHK saptandı. Burunda yerleşen BHK’u ele alan geniş olgu çalışmalarda burun deliklerinin tutulumuna çok ender rastlandığı görülmektedir. Bu durumun nedeni tam olarak bilinmemektedir, ancak göreceli olarak düşük ultraviyole ışık ve henüz tam olarak tanımlanmamış başka faktörlerin etkili olması muhtemeldir
